Transition Services
Growing up is not easy! It is even more complicated for young adults with disabilities. In the past, far too many students with disabilities left school lacking the academic, social and technical skills they needed to succeed in the adult world. Both Congress and the U.S. Department of Education recognized the need for early transition planning and included it in the Individuals with Disabilities Education Act (IDEA). Northwest Colorado BOCES emphasizes the implementation of transition services in each of its member districts’ middle and secondary schools.
Careful transition planning beginning at the earliest age possible will prepare students to move from school to community living and employment. Good program planning can help them become independent, productive adults.
Transition services include a coordinated set of activities for a student, designed within an outcome oriented process, that promotes movement from school to post-school activities, and may include any of the following:
• Employment (including supported employment)
• Post-secondary education
• Vocational training
• Continuing and adult education
• Adult services
• Independent living
• Community participation
A coordinated set of activities is based upon the individual student’s needs, taking into account the student’s preferences and interests and includes:
• Community experiences
• The development of employment
• The development of other post-school adult living activities
• When appropriate, acquisition of daily living skills
• When appropriate, functional vocational evaluation
